How can I send someone a friend request if there myspace is private and you have to know there Last Name or Email Address to send them a request and I don't know either, and I can't ask them?

Try to find a mutual friend that you have on myspace, and ask them to help you out. They should be listed in someone's friend list if they are also friends with them, and that way that person can either help you out with the last name or email address, or ask the private member to friend request you.

Then you are pretty much out of luck, the only way is sending them a message asking.
