Do it from iTunes. With mine, I click the song and copy and paste whatever picture I like (usually the album artwork) into the space for pictures in the bottom left corner. Then I sync away.
Do you remember how you put the picture on there? It's exactly the same.. :)

I would suggest you plug your IPOD into itunes and go to the song that has your ex's picture on the music list on the itunes page, then go to file then click GET INFO and go along to the last tab in the window that says artwork and delete the picture ( this should just be as simple as clicking it and pressing the delete button on your keyboard) then close the window and SYNC your ipod again so that the iPOD can re-sync without the picture being there. ITS VERY IMPORTANT TO REMEMBER TO RE-SYNC your iPOD again. I hope this helps you. :)
