Read the documentation carefully. You need to:
-Download custom.zip
-Modify helpdesk.txt to your desired settings
-Insert your own icons/logo (optional)
-Zip all of the files into a new custom.zip
-Upload to the UltraVNC SC Creator tool
-Download the EXE
-Forward the port you used in helpdesk.txt (default 5500)
-Install UltraVNC on your system
-Run UltraVNC in listen mode
-Have the person you're helping run your EXE

The syntax of helpdesk.txt is explained on the UltraVNC SC page linked above. I use DynDNS to make theymos.ath.cx point to me:

Add [DIRECT] immediately before [HOST] to make the connection start immediately after your EXE runs, without a menu. I used to do this, but some people weren't entirely prepared for it and it freaked them out. [ theymos's advice column | Ask theymos A Question ]
