What you need to do is find a planned parenthood or women's clinic. It's much more friendlier and you'll feel more comfortable than if you went to your regular doctor's office. You don't have to be a certain age, and they're very exclusive when it comes to privacy. They deal with this all time. 2. No you don't unless you really want to and unless the provider requires parental guidance.
3. No you don't have to be a regular patient, although some would require you to be for later check-ups on the Pill.
