I don't know what kind of doctor's apointment it is, but if its just a pediatrition and you're getting a check up, my doctor lets me wear a pair of shorts (so it doesnt matter whether i wear a tampon or not) and a tank top instead of the paper dress. and yes, of course you can still wear a bra.
but be prepared to take it off if you're going for a specific problem where he/she may need to examine you in that place.
