Makes use of the tongue... a lot. I can't speak from personal experience on this one however, so all I really know is that its the tongue doing the work. Not a lot of stuff grosses me out really, but this seems to hit the jackpot on the gross level to me so its agonizing for me to answer this. Anyways, he uses his tongue and I guess explores (k seriously this is killing me to even type). Theres probably some hand action thrown in there to mix things up and using the rest of the mouth besides the tongue, which would be the lips.
Be grateful, that was not easy for me to answer heh.
