What does an abortion consent form from a parent have to have on it in the state of texas?

From what I know, in the state of texas, your parents must be notified 48 hours before your abortion. I don't think they have to give consent to it, they just have to be notified.
If the law is changed, which I don't think it is, a consent form would most likely require their signature.
