First things first. Poems - unless you're a true poet at heart and she's a literary fiend - are a no-no. Why? One word: Corny. Most of the time.
But then again, if she likes you back, whatever you do, she'll probably think it's awfully nice.
So my advice is that you should do whatever you feel most comfortable with. If you write her this magnificent, 20 pages long poem, but can't even face her to give it to her, it probably isn't the good solution.
You don't have to do something utterly romantic. Just be yourself and everything (hopefully) will go just fine.
